Terms of Service
Draft, pending attorney review. Effective October 6, 2026.
The short version
- Instant EOB explains what your documents say. It is not legal, medical, insurance or financial advice.
- The AI can make mistakes. Check anything important with your insurer or provider before you act on it.
- You decide what to pay and when. Payment and appeal deadlines are yours to keep.
- Only upload documents you're allowed to share.
1. Agreeing to these terms
These terms are an agreement between you and [owner's legal name], a sole proprietor in Nebraska, USA, who operates Instant EOB ("we," "us"). By using Instant EOB, you agree to these terms and to our Privacy Policy. If you don't agree, please don't use it. You must be 18 or older to use Instant EOB.
2. What Instant EOB is, and what it isn't
Instant EOB is an educational tool. It reads documents you upload, such as Explanation of Benefits statements and medical, dental or vision bills, and explains in plain English what they say. It may point out things you might want to ask about.
Instant EOB is not:
- legal, medical, insurance, financial or tax advice;
- a lawyer, health care provider, insurance company, insurance agent or broker, public adjuster, billing advocate or debt counselor;
- affiliated with, endorsed by or acting for any insurer, health plan, provider or government agency.
Using Instant EOB doesn't create any attorney-client, provider-patient, fiduciary or agency relationship. For advice about your situation, contact your insurer, your provider, your state insurance department, a licensed professional or a patient advocate.
3. The AI can be wrong
Reviews are created by artificial intelligence. It can misread a page, miss information, get numbers or codes wrong, or describe something incorrectly, especially with blurry photos, cut-off pages or unusual formats. A review that finds nothing to look at doesn't mean nothing is wrong. Always compare a review against your original documents, and confirm anything important with your insurer or provider before relying on it.
4. Your decisions are yours
- Instant EOB never tells you whether to pay a bill. Whether, what and when to pay is your decision.
- You are responsible for meeting every deadline, including payment due dates and appeal or dispute deadlines. Using Instant EOB doesn't pause or extend any deadline.
- Not paying a bill can lead to late fees, collections or credit reporting. Talk to the provider before withholding any payment.
- We don't guarantee any outcome, refund, correction or savings.
5. Not for emergencies
Instant EOB is not for medical emergencies or urgent decisions about care. In an emergency, call 911.
6. Your documents
- You promise you have the right to upload each document: it's yours, a dependent's, or you have permission or legal authority from the person it's about.
- You keep ownership of everything you upload. You give us permission to process it only to create your review, as described in our Privacy Policy. That permission ends when your review is done.
- Don't upload anything unrelated to health care billing, anything you know is false or altered, or anything you're not allowed to share.
7. Acceptable use
You agree not to:
- use Instant EOB for anything illegal, fraudulent or harmful;
- upload another person's documents without permission or legal authority;
- use automated tools to access it, or send more requests than a person reasonably would;
- try to break, overload, probe or get around its security or limits;
- copy, resell or reverse-engineer it, or use its output to build a competing service or dataset;
- use it to create fake or misleading documents.
We may block access for anyone who breaks these rules.
8. Medical codes
CPT® codes and descriptions are owned by the American Medical Association, and CDT codes by the American Dental Association. Instant EOB shows these codes only as they are printed on your documents and doesn't explain them unless and until it holds the required licenses. Nothing in Instant EOB gives you a license to any code set.
9. Our content
Instant EOB, its design, text, software and name belong to us. You may use your reviews for your own personal purposes, including sharing them with your insurer, provider, family or advisors. If you send us feedback or ideas, we may use them without owing you anything.
10. Price
Instant EOB is free today. If we add paid features, the price and any extra terms will be shown clearly before you pay, and you will never be charged without agreeing first.
11. Availability and changes
We may change, pause or stop Instant EOB, or any part of it, at any time. It may sometimes be slow or unavailable. Please keep your original documents; we don't store them.
12. Other services
Instant EOB relies on Anthropic (AI processing) and Render (hosting), and may link to other websites such as government resources. We don't control those services and aren't responsible for them.
13. No warranties
Instant EOB is provided "as is" and "as available." To the fullest extent the law allows, we disclaim all warranties, express or implied, including warranties of accuracy, completeness, merchantability, fitness for a particular purpose and non-infringement. We don't promise that Instant EOB or any review will be accurate, complete, error-free, secure or always available.
14. Limits on our liability
To the fullest extent the law allows, we are not liable for any indirect, incidental, special, consequential or punitive damages, or for any lost money, savings, payments, late fees, collection costs, credit harm or missed deadlines arising from your use of Instant EOB or your reliance on a review. Our total liability for any claim related to Instant EOB is limited to the greater of the amount you paid us in the 12 months before the claim, or fifty U.S. dollars ($50).
Some places don't allow some of these limits, so they may not all apply to you. Nothing in these terms limits rights you have that the law says can't be limited.
15. Your responsibility
If you break these terms or misuse Instant EOB, including by uploading documents you weren't allowed to share, you agree to cover the reasonable costs and losses that result for us, including reasonable legal fees, to the extent the law allows.
16. Disputes
If you have a problem with Instant EOB, please email [contact email] first and give us 30 days to try to resolve it informally. These terms are governed by the laws of the State of Nebraska, without regard to conflict-of-law rules. Any dispute that isn't resolved informally will be handled in the state or federal courts located in Douglas County, Nebraska, except that either of us may bring a claim in small claims court where it qualifies.
17. Ending use
You can stop using Instant EOB at any time. We may suspend or end access for anyone who breaks these terms. Sections 3, 4 and 8 through 18 continue to apply after that.
18. General
- Changes to these terms: we'll post any new version here with a new effective date. Using Instant EOB after a change means you accept it. Changes don't apply to disputes that started before the change.
- Whole agreement: these terms and the Privacy Policy are the entire agreement between you and us about Instant EOB.
- Severability: if any part is found unenforceable, the rest still applies.
- No waiver: if we don't enforce a term, we can still enforce it later.
- Transfer: you can't transfer these terms. We may transfer them if Instant EOB is sold or reorganized, for example into a company we form.
